Under the National Tuberculosis Elimination Programme (NTEP), a patient diagnosed with pre-XDR-TB (previously XDR-TB) is defined as having resistance to:
- A Rifampicin and isoniazid only (MDR-TB)
- B Rifampicin plus all second-line injectables and at least one fluoroquinolone
- C Rifampicin plus any fluoroquinolone, plus bedaquiline or linezolid or both ✓
- D Rifampicin, isoniazid, and at least three Group B drugs
Explanation
In 2021, WHO revised XDR-TB classification: XDR-TB now requires MDR/RR-TB plus resistance to any fluoroquinolone AND at least one of bedaquiline or linezolid. Pre-XDR-TB (the former XDR-TB definition) was MDR/RR-TB resistant to any fluoroquinolone plus resistance to bedaquiline or linezolid. Under NTEP's current framework, these patients receive a longer BPaLM or BPaL-based regimen. The old definition requiring injectable resistance is obsolete.
