Explanation
Contralateral hemiballismus classically follows a lesion of the subthalamic nucleus, most often a small haemorrhage from the posterior communicating or posterior cerebral artery perforators. Substantia nigra lesions produce parkinsonism rather than ballism, and pure thalamic lesions give sensory loss.
