Donovanosis (granuloma inguinale) is caused by Klebsiella granulomatis. The characteristic histopathological finding is:
- A Owl-eye intranuclear inclusions in fibroblasts
- B Schaumann bodies within epithelioid granulomata
- C Donovan bodies — intracytoplasmic bipolar-staining inclusion bodies within large mononuclear cells (Mikulicz cells) ✓
- D Reed-Sternberg-like giant cells with interstitial inflammation
Explanation
Donovanosis shows characteristic Donovan bodies — encapsulated bacteria (K. granulomatis) seen as intracytoplasmic inclusions within large vacuolated mononuclear cells called Mikulicz cells, stained with Giemsa or Leishman stain. The bacteria appear as bipolar ('safety pin') staining organisms. The disease is characterized by beefy-red painless ulcers without lymphadenopathy. Treatment is with azithromycin or doxycycline until complete healing.
