Necrobiosis lipoidica diabeticorum (NLD) characteristically presents as:
- A Bullae on dorsum of feet after minor trauma
- B Discrete papules of xanthoma on Achilles tendon
- C Yellowish-red plaques with central waxy atrophy and telangiectasias on the shins ✓
- D Pruritic hyperpigmented macules in flexures
Explanation
Necrobiosis lipoidica diabeticorum presents as well-demarcated yellowish-red plaques with central waxy (porcelain-like) atrophy and prominent telangiectasias, classically on the pretibial area. Histologically it shows layered palisading granulomas with necrobiotic collagen. About 65% of cases occur in diabetics, but only ~0.3% of diabetics develop NLD. Bullosis diabeticorum is the non-scarring blistering disorder of diabetics; tendon xanthomata reflect familial hypercholesterolaemia.
