Pathology · Lung Pathology (Obstructive, Restrictive, Tumors, Infections)

A previously healthy 22-year-old man converts his tuberculin skin test to positive. Chest radiograph shows a 1.5 cm subpleural nodule in the right mid-lung zone with ipsilateral hilar lymphadenopathy. This combination constitutes:

  • A Ranke complex
  • B Simon focus
  • C Ghon complex
  • D Assmann focus
Correct answer: C. Ghon complex

Explanation

The Ghon complex is the lesion of primary tuberculosis: a peripheral parenchymal focus (Ghon focus), usually subpleural in the upper part of a lower lobe or lower part of an upper lobe, together with involved draining hilar lymph nodes. With healing and calcification it becomes the Ranke complex. The Simon focus is a hematogenous apical lesion, and the Assmann focus is the apical lesion of secondary TB.
