Obstetrics & Gynaecology · Prolapse, Urinary Incontinence and Fistulas

A 62-year-old postmenopausal woman has uterovaginal prolapse. On examination, the cervix is 2 cm above the hymen at maximum Valsalva. The posterior vaginal wall bulges 1 cm beyond the hymen. Using the POP-Q (Pelvic Organ Prolapse Quantification) system, the cervix position would be coded as which value?

  • A Point C = –2
  • B Point C = +2
  • C Point Ba = +1
  • D Point Bp = –1
Correct answer: A. Point C = –2

Explanation

In the POP-Q system, measurements are taken in centimetres relative to the hymen: positive values indicate positions beyond (distal to) the hymen; negative values indicate positions proximal (inside) the hymen. Point C = cervix or vaginal vault apex. A cervix 2 cm above the hymen = –2 cm (inside/proximal). Point Ba = most distal point of anterior vaginal wall; Point Bp = most distal point of posterior vaginal wall. The posterior wall 1 cm beyond hymen = Bp = +1. Stage II prolapse is defined as leading edge between –1 and +1 cm relative to the hymen.
