Narrow-band UVB (NB-UVB, 311–313 nm) has largely replaced broadband UVB for phototherapy. NB-UVB is the first-line phototherapy for all of the following EXCEPT:
- A Vitiligo
- B Plaque psoriasis
- C Palmoplantar psoriasis (palmoplantar involvement) ✓
- D Mycosis fungoides (early stage)
Explanation
Palmoplantar psoriasis is a specific indication where NB-UVB is LESS effective due to the thick stratum corneum of palms and soles limiting UV penetration. For palmoplantar disease, PUVA (psoralen + UVA)—particularly soak PUVA or paint PUVA—is preferred. NB-UVB is first-line phototherapy for vitiligo, plaque psoriasis, and early-stage mycosis fungoides (patch/plaque stage, IA-IIA). PUVA is also used for CTCL but NB-UVB is increasingly favoured for early stages.
